What essentials do we need to train our church bus driver on for handling an accident or breakdown during a long-distance ministry trip?

Is there a sample transporation waiver form anywhere online that our church could use for the occasional youth choir transporaiton to local churches within a 50 mile radius?
I am the assistant youth choir director and we do have engagments for youth day programs and church service through out the year and want to make sure that our youth remain safe and covered while in our care.
We are a small methodist church located in TN and could use some insight on this subject as we are usually the two adults transporting these children in 2-3 cars and we both have liability and full coverage through our own personal insurance companies.
Is there a waiver for parents to sign giving the youth permission to travel on these events while at the same time, releaving us (the directors) of any liability? Thank you
